Contraindications

- Hypersensitivity to the active substance(s) or to any of the excipients.

- Hypercalcemia and/or hypercalciuria.

 -Nephrolithiasis and/or renal calcification

- Severe renal failure.

- Hypervitaminosis D.

- Pseudo-hypoparathyroidism in which the need for vitamin D may be reduced due to the normal phases of vitamin D sensitivity, which carries a risk of prolonged overdose.

Drug interactions

- Concomitant use of anticonvulsants (eg phenytoin) or barbiturates (and possibly other drugs that induce hepatic enzymes) may reduce the effect of vitamin D3 by disrupting the metabolism.

 - In cases of treatment with thiazide diuretics that reduce urinary calcium excretion, monitoring of serum calcium concentration is recommended.

- The simultaneous use of glucocorticoids can reduce the effect of vitamin D3.

- In cases of treatment with drugs containing digitalis and other cardiac glucosides, the

- Taking vitamin D3 may increase the risk of digitalis poisoning (irregular heartbeat).

- Strict medical supervision is required, along with serum calcium concentrations and ECG monitoring if necessary.

- Concomitant treatment with an ion exchange resin such as cholestyramine, colestipol hydrochloride, orlistat or a laxative such as paraffin oil may reduce gastrointestinal absorption of vitamin D3,.

- The cytotoxic agent actinomycin and imidazole antifungal agents interfere with vitamin D3 activity by preventing the conversion of 25-hydroxyvitamin D3 to 1,25-dihydroxyvitamin D3 by the kidney enzyme (25-hydroxyvitamin D3-1 hydroxylase.)

Dosage and usage

 

Vitamin D (ergocalciferol-D2, cholecalciferol-D3, alfacalcidol) is a fat-soluble vitamin that helps the body absorb calcium and phosphorous. Getting the right amount of vitamin D, calcium, and phosphorous is important for building and maintaining strong bones. Vitamin D is used to treat and prevent bone disorders (such as rickets and osteomalacia).

- Vitamin D is used with calcium to treat or prevent bone loss (osteoporosis). Vitamin D is also used with other medications to treat low calcium or phosphate levels caused by certain disorders (such as hypoparathyroidism, pseudohypoparathyroidism, familial hypophosphatemia).

Dosages in children:

  •       For the prevention of deficiency (10-18 years): one capsule of 25,000 IU every 6 weeks.

  •        Deficiency treatment (10-18 years): 25,000 IU (1 capsule) once every 2 weeks for 6

     Weeks (followed by a maintenance treatment of 400-1000 IU/day, such as 1 capsule per month.

Adults: 

  •       Prevention of Vitamin D deficiency: 25,000 IU/month (1 capsule). Higher doses may be required in certain situations.

  •       As an adjunct to the specific treatment of osteoporosis: 25,000 IU/month (one capsule)

  •        Treatment of Vitamin D deficiency (<25 nmol/L) 50,000 IU/week (2 capsules) for 6-8

    weeks, followed by maintenance therapy (1400-2000 IU/day may be required, eg two capsules per month; follow-up (OH)D 25 measurements should be made approximately three to four months after initiation of follow-up treatment to confirm that the target level has been achieved

Side Effect

-       Renal: Impaired renal function with frequent urination, nocturia, polydipsia, hypercalciuria, reversible azotemia, hypertension, renal calcification, generalized vascular calcification, or irreversible renal failure may result in death.

-       Central nervous system: mental retardation.

-       Soft tissue: diffuse calcification of the soft tissues, including the heart, blood vessels, renal tubules, and lungs.

-       Skeletal: Bone purification (osteoporosis) in adults occurs concurrently.Decrease in average linear growth rate and increased bone mineralization in infants and children (dwarfism), vague aches, stiffness, and weakness

-  Digestive system: nausea, loss of appetite, constipation
