Chemical composition

Each rectal suppository contains 60, 125, 500 mg ibuprofen.

Contraindications

-          Hypersensitivity to the active substance or to any of the excipients

-          Gastrointestinal disorders: ulceration or bleeding, Crohn's disease, ulcerative colitis,

-          Kidney failure or any liver impairment

-           High blood pressure, heart failure, or congestive failure.

-       A history of bronchospasm, asthma, rhinitis, angioedema or urticaria associated with use of ASA or other NSAIDs

Drug interactions

- Increased risk of gastrointestinal ulcers and bleeding with: NSAIDs, oral dicoumarin anticoagulants, ASA-type antiplatelet agents, oral corticosteroids, and selective serotonin reuptake inhibitors.

- Reduces the effectiveness of: furosemide and thiazide diuretics.

Reduces the hypotensive effect of: beta-blockers, ACE inhibitors, and angiotensin II antagonists.

- Risk of hyperkalemia with: potassium-sparing diuretics.

- Reduces the effect of: Mifepristone.

- Increases plasma levels of: digoxin, phenytoin, lithium.

- Increases toxicity: methotrexate, hydantoin, sulfa drugs, baclofen.

- Strengthens gastrointestinal lesions with: salicylates, phenylbutazone, indomethacin and other NSAIDs.

- Increases the effect of: oral hypoglycemic agents and insulin.

- Additive effect on platelet inhibition with: ticlopidine.

- Increased risk of hematological toxicity from: Zidovudine

Dosage and usage

 

- It is used as an analgesic for mild to moderate pain: headache, toothache, and as an antipyretic.

·         Adults: One 500 mg suppository 2-3 times daily, provided that the total dose does not exceed 2400 mg in divided doses.

- Iburect suppositories are mainly used in children of 3 months of age who weigh at least 6 kg

- The total daily dose of ibuprofen is 20-30 mg/kg, according to weight, in 3 or 4 single doses.

- 6-8 kg (3-9 months): one 60 mg suppository per day, to be repeated if necessary after 8 hours.

- 8 -12.5 kg (9 months to 2 years): one suppository every 6 hours, not to exceed 4 suppositories per day.

·         Hepatic impairment: Dosage adjustment in mild hepatic impairment and should not be given in patients with severe hepatic impairment.

·         Renal impairment: Reducing the dose in mild to moderate renal impairment, and contraindicated in severe renal impairment.

Side Effect

Peptic ulcer, gastrointestinal perforation and bleeding, nausea, diarrhoea, flatulence, constipation, dyspepsia, abdominal pain, hematemesis, ulcerative stomatitis, exacerbation of ulcerative colitis, and ulcerative colitis.
